Overview Of All On 4

All on 4
What Is All On 4?

All-on-4 is a revolutionary dental restoration technique for patients who have lost all of their teeth. It was developed by Nobel Biocare (Sweden) and first introduced by Dr. Paulo Malo in 1998.

  • Meaning of the name: “All” refers to all teeth in one arch being supported “On 4” dental implants.

  • Nature of the technique: Instead of placing an individual implant for every missing tooth (which is costly and invasive), surgeons place only 4 implants per arch to act as a foundation. A fixed bridge of 12-14 teeth is then securely attached to these 4 implants.

Who is it for?

This method is typically recommended for:

  • Full edentulism: People who have lost all teeth in the upper arch, lower arch, or both.

  • Severely damaged teeth: Patients with remaining teeth that are loose or suffering from severe periodontal disease that cannot be saved.

  • Jawbone resorption (Bone loss): Patients who have lost bone in the posterior (back) area and want to avoid complex bone grafting procedures required by traditional implants.

  • Denture wearers: Those who find traditional removable dentures uncomfortable, loose, or painful.

The All-on-4 Technique

The uniqueness of All-on-4 lies in the specific positioning and angulation of the implants:

  • Implant Positioning:

    • 2 Front Implants: Placed vertically in the anterior (front) region (usually at the position of the lateral incisors).

    • 2 Rear Implants: Placed in the posterior (back) region and tilted at an angle of up to 45 degrees

Tilting the back implants allows the surgeon to utilize the higher-quality bone available in the front of the jaw. This avoids critical anatomical structures like the maxillary sinuses (in the upper jaw) or the mental nerve (in the lower jaw), eliminating the need for bone grafts.

Specialized connectors (abutments) are used to compensate for the implant angles, ensuring that the final bridge sits straight and provides a balanced bite.

Immediate Final Teeth™: Strength Meets Artistry

At the heart of the Success Code® Protocol is the Immediate Final Teeth™ system. Unlike temporary solutions, this approach utilizes a high-strength titanium framework custom-milled to fit perfectly.

  • Rapid Delivery: Your new teeth are ready in as little as 24 hours.
  • Maximum Durability: The titanium frame connects all implants together, spreading biting pressure evenly and protecting each implant during the critical healing phase.
  • Hand-Crafted Aesthetics: Our master technicians hand-design each tooth to ensure a natural, premium look that matches your facial features.
  • Easy Maintenance: Once the foundation is stable, the bridge is designed for easy adjustment or replacement in the future if ever required.

Structural Harmony: Bone & Gum Reshaping

A key element of the Success Code® is the meticulous preparation of the treatment site. During surgery, we perform precise reshaping of the jawbone and gum tissue.

  • Foundation Smoothing: We even out any rough or bumpy bone areas left by missing teeth, creating a flat, cleanable surface.
  • Accelerated Healing: A smooth, reshaped gum line promotes faster healing and prevents tissue shrinkage over time.
  • Natural Tooth Alignment: This reshaping creates the necessary space to integrate a natural-looking gum part into the new teeth, allowing for a more authentic and youthful smile.
  • Self-Healing: All surgical stitches used in this protocol are designed to dissolve naturally within approximately 3 weeks.

Digital Precision: Exact Implant Positioning

The Success Code® Protocol relies on advanced digital planning to eliminate guesswork. We utilize developed surgical protocols to ensure that every implant is placed with mathematical accuracy.

  • Optimal Spacing: Using 3D imaging and digital guides, we position each implant in the exact spot to maximize bone support.
  • Long-Term Reliability: Perfect spacing and depth create a stable foundation that protects your investment, making the final restoration more reliable and durable for the long term.
  • Improved Cleanability: Precise positioning ensures that your fixed bridge is easy to brush and maintain, reducing the risk of future clinical issues

Frequently Asked Questions

How long do All-on-4 implants last?

The implants (the screws) are designed to be permanent and can last 20 years to a lifetime with proper care. The prosthetic bridge (the teeth) undergoes wear and tear and may need replacement or repair every 10–15 years.

What is the difference between All-on-4 and All-on-6?

All-on-4 uses 4 implants, while All-on-6 uses 6. All-on-6 provides more stability and distribution of pressure, but it requires more bone volume. All-on-4 is often preferred for patients with significant bone loss.

Why choose All-on-4 over traditional dentures?

Unlike dentures, All-on-4 is fixed (doesn’t slip), allows you to eat almost any food, prevents bone loss, and does not require messy adhesives or daily removal.

When can I eat normally again?

For the first 3–4 months, you must stick to a soft food diet (pasta, fish, eggs, cooked vegetables). This is the “osseointegration” period where the implants fuse with your bone. Once the final permanent bridge is attached, you can eat steak, apples, and crunchy foods.
